Callsign Clubleaf 212 was shot down on 21 January 1991 during OPERATION DESERT STORM. The RIO, Lt Lawrence 'Rat' Slade was taken POW. The Pilot, Lt Devon Jones, was rescued by Spec Ops Forces following a nine (9) hour rescue mission.
